Physical Examination

Students registering for Athletics must have a current physical examination on file in the Activities office before the student is authorized to participate in practice. To ensure clearance for participation, physicals should be completed and turned in to the Athletics office prior to the first day of practice/tryouts.

Physicals are to be renewed on or after May 1st preceding the school year for which it is applicable.

Download Preparticipation Physical Form HERE. Families only need to have the Physician portion of this form completed via hard copy to upload. All other information is gathered during the registration process. Only page 3 and page 4 are required to be completed by your selected physician in order to upload the document in the physical section of registration.

Academic Record

Student who wish to participate with Christ Prep Athletics are required to upload their previous semeter's ACADEMIC RECORD/TRANSCRIPT to verify eligibility. These records should indicate the student athlete is meeting the minimum academic requirements for graduation per the Kansas Department of Education standards. Additional academic record information may be requested by CPA if it is unclear that a student athlete is on a path to graduation from highschool.

*Registration Fee

• $50/Athlete Registration/Sport fee per student athlete. CPA Academic students have already paid this with their enrollment fee. Enrollment fee is non-refundable.

*Sport Tuition
• Each sport is individually priced and will be indicated when you reach the end of registration. IF you have any questions about the sport specific tuition, please check with Christ Prep Administrative staff. Sport Tuition is Non-Refundable.

*Uniform Deposit
• CPA Sports Programs require a $100 Uniform Deposit Fee; this amount must be paid separately from any other payment: it will be held by the school and returned to you once the uniform is returned at the end of the season in a clean and undamaged condition.

All checks can be made payable to Christ Preparatory Academy (or CPA) - write in the memo what the payment is for.
